• The purpose of the discharge hub is to help people to leave Ealing Hospital as soon and as safely as possible to the right place to support their recovery.

  • Jasmin ward provides general rehabilitation in a community-based setting. We aim to help patients improve what they can do and achieve their goals for rehabilitation, through nurse and therapy led care.

  • We provide pharmacy services at Clayponds Hospital, Broadmoor Hospital and the St Bernard’s Hospital site.The pharmacy service supports staff, patients and carers to use medicines safely and effectively.

  • Our joint Rapid Response and Home First teams provide a crisis response for people who are at risk of hospital admission and help people return home quickly after a severe or sudden illness when they’ve been discharged from a general hospital.

  • The team on Magnolia ward helps people who’ve had a severe or sudden illness so they can recover in a calm and welcoming environment and avoid being admitted to a major hospital.

  • Rosemary Ward provides specialist and neurological rehabilitation in a community-based setting which aims to help patients improve what they can do and achieve their goals for rehabilitation through multidisciplinary care.
