The team on Jasmin ward provides general rehabilitation in a community-based setting. We aim to help patients improve what they can do and achieve their goals for rehabilitation through nurse and therapy led care.

- Daily menu catering for specialist diets
- Dietician
- Medical cover. Support from the medical team on site every day, with on-call support from our primary care team
- Mental health nurses
- Occupational therapists
- Physiotherapists
- Psychologists
- Rehabilitation assistants
- Specialist rehabilitation nurses
- Speech and language therapists
- Translation service.
Patients are nursed in single sex bays of 6 to 8 beds.
Single side rooms are available for those patients who require specialist or isolation nursing care.
We have a large, well-equipped gym, a therapy kitchen and bathroom in which to practice daily activities before going home.

3:00pm - 5:15pm and 6:30pm - 7:15pm
Up to 2 visitors are allowed per person.
Ward routine
Breakfast: 8am
Lunch: 12.30pm
Dinner: 5.30pm
Patients can be referred to Jasmin Ward if they require rehabilitation that can only be provided in an inpatient setting. Referrals are usually made by the ward therapy team of an acute hospital. However, patients receiving community rehabilitation may also be referred if they require a brief period of intense treatment. The referrer should ask for your consent for you to take part be in a community-based inpatient rehabilitation programme.
If you agree, they’ll send a referral to the Jasmin Ward team. If you’re already known to Ealing Community Partners, they can also share your care plan.
If you’re eligible for the service, we’ll aim to offer you an admission within 24 hours. If a suitable bed isn’t available, we may pass your referral to our partner community organisations in neighbouring boroughs.
