Community team for people with learning disabilities
Adults with a diagnosis of learning disability can get support from the community team for people with learning disabilties (CTPLD).
This is a team of health and social work professionals. They work in partnership with people with learning disabilities, their families, their carers and other agencies to overcome difficulties and problems.
The team is made up of different professionals including:
- Social workers
- Speech and language therapists
- Physiotherapists
- Learning disability nurses
- Clinical psychologists
- Consultant psychiatrist
- Advanced nurse practitioners
- Positive behaviour support practitioners.
To access the health service, adults with a learning disability must have an Ealing GP.
To access the social services in the team, adults with a learning disability must have a residential address in Ealing.
Referrals can be made by professionals. Self-referrals are also welcomed.
