The Trust oversees delivery of:
- A comprehensive portfolio of community health services for children and adults in Ealing (Ealing Community Partners)

Intermediate care services in Hammersmith and Fulham in partnership with trusted providers
- All levels of mental health support for children, adults and older adults in Ealing, Hammersmith and Fulham and Hounslow.
During its first year, Ealing Community Partners (ECP):
- Established a 24/7 community nursing service
- Set up a 24/7 Ealing community referral hub
- Delivered improvements to district nursing teams
Priorities for transforming services include:
- Developing integrated primary care network teams, enabling the alignment of physical and mental health services around primary care networks (local groups of GPs working together)
- Enhancing the Ealing community referral hub to deliver a single point of access for all Ealing community health services
- Joining up multiple small specialist rehabilitation teams into a flexible adult rehabilitation service which supports discharge from acute hospital patients to regain their independence. This also involves better alignment with child and adolescent mental health services.
- Delivering enhanced primary care services for nursing home residents, including supporting training and development of social care staff, and direct delivery of physical, mental health and end of life care services
- Working closely with primary care providers and the clinical directors of primary care networks, to recruit a range of additional roles from social prescribers, first contact physiotherapists, clinical pharmacists and mental health workers.
