The community referral hub is a single point of contact and referral for all services provided by Ealing Community Partners (ECP).
A team led by clinicians brings together all ECP services into one single team for administrative triage, 24 hours a day, 7 days a week.
The community referral hub welcomes referrals for eligible patients from healthcare professionals.
We can also help with enquiries directly from patients and carers who are using a service.
