The Trust provides a GP service for care homes in the Ealing area. We provide routine medical and urgent GP care 7 days a week, and 365 days a year.
The service looks after nearly 40 nursing and residential homes across the borough of Ealing ranging from smaller homes of 22 beds to the largest home of 165 beds.
We look after all age ranges, end of life patients, dementia patients, patients with co-morbidities and patients with learning disabilities.
Each care home receives dedicated visits from a multidisciplinary team, we provide seasonal vaccinations, a repeat prescription service, medication monitoring and regular medical reviews to all registered residents.
Care home staff can use the calls to discuss:
- Deteriorating residents
- Residents who need advanced care plans or new prescriptions
- Residents being moved in or out of hospital
- Other areas of concern, such as unwell staff or personal protective equipment.
The enhanced care team may be able offer clinical advice on the spot or arrange for prompt support from the resident’s GP, pharmacist or community services.
The team also coordinates advice and support from other Ealing Community Partners services to ensure that all care home residents have access to the same services as people who live in their own homes, and that training and support is provided to staff working in the care sector.
A dedicated advanced practitioner in infection prevention and control provides dedicated expert advice and support to social care settings.
