This service is for adults with new or long term mental health needs who need extra support to help them recover, but do not need to be referred to specialist services.

We also support people who are being discharged from secondary care mental health services, such as a community mental health teams, crisis assessment and treatment teams, or inpatient wards. 

We’re currently developing and expanding the service and working more closely with Primary Care Networks and secondary care services, to become Mental Health Integrated Network Teams (MINT).

​Our primary care team includes: 

  • ​Nursing staff 
  • A consultant psychiatrist 
  • Psychologist 
  • Administrator.

The team works to support your GP and primary care team to offer you the help you need. It aims to prevent the need for referral (or re-referral) to secondary care by helping to stop your mental health getting worse again.

Care is provided across sites in Ealing, including GP practices and in patients' homes.
